What is Fast Track funding?

The Fast Track pathway tool is used where a doctor or nurse considers a person should be fast tracked for NHS Continuing Healthcare funding because the person has a rapidly deteriorating condition that may shorten their life. The person may need NHS Continuing Healthcare funding to enable their needs to be met either in their own home or in a care setting. You will receive a letter from NHS Warrington Clinical Commissioning Group explaining the process once the funding has been approved.

How long does the funding last for?

NHS Warrington Clinical Commissioning Group will complete a review of needs usually at six weeks and again at 12 weeks.

There may be some situations where the Fast Track process is later found to have been inappropriate, for example because the decision was made after an acute episode of a condition which was subsequently found to be treatable or your condition stabilises.

However, no one who has been identified through the Fast Track process as eligible for NHS Continuing Healthcare should have their support removed without their eligibility being reviewed in accordance with the review processes set out in the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care.

What does the review involve?

The review should include completion of a decision support tool by a multidisciplinary team which will make a recommendation on eligibility.

The person/family/representative will be invited to contribute to the review.

You will receive a letter confirming the outcome from NHS Warrington Clinical Commissioning Group.

What about the care home fees?

If the Fast Track funding is stopped, you may be liable for the cost of or contribution to your care home placement. Warrington Borough Council will be able to offer further advice on this.

Disputing decisions

If you wish to dispute the outcome of the decision support tool, details of how to appeal will be included in the outcome letter that will be sent to you to enable you to follow NHS Warrington Clinical Commissioning Group’s complaints process.
