Connect Yourself (Pilot)

Warrington Borough Council and Warrington Speak Up have been successful in securing  funding from NHS Transforming Care to pilot a new community based service for young people and adults from the age of 18 years with special educational needs and/or disabilities (SEND), including those with learning disabilities and/or autism. ‘Connect Yourself’ will support young people and adults at risk of social exclusion to identify and link them to a range of opportunities within Warrington which promote:

• Access to employment

• Independent living

• Community inclusion

• Good physical health

• Improved emotional wellbeing.

The service will adopt a person-centred approach and each intervention will be shaped by the individual choices of the young person. The service will be delivered by Warrington Speak Up. The project links to a number of other initiatives being undertaken across Warrington to strengthen the Preparation for Adulthood local offer, helping young people and adults to access services and meaningful experiences and to increase the proportion of disadvantaged adults in paid employment.

Eligibility criteria:

  • From 18 to 25 years olds
  • Last year of education or have finished education with no agreed destination
  • Known to the Transition Team or completing a Supported Internship
  • Referred by a professional with consent of the individual and parent/carer
  • Must have a confirmed learning disability and/or autism
